New Patient Registration

moterh_and_babyThe doctors welcome new patients who live within our practice area. 

To register as a patient please ask at reception for details. Newly registered patients are invited for a new patient consultation with the practice nurse.  Once registered, please keep us informed of any name and address changes.  In particular, we would like to be able to contact patients by SMS text messaging.  Please let us know your mobile number.

If you wish to choose to see a particular doctor for some or all medical conditions, please let the receptionist know and she will note this on your medical records. We will always try to ensure that this is possible, but there may be occasions when it is not possible.

All new patients are asked to provide photographic indentification when registering.  Such ID includes Driving License, passport or local bus pass.
